Summary
Gilteritinib can prolong the QT interval, and fluoxetine can add to this effect, raising the risk of torsades de pointes. Gilteritinib's activity may also be affected by drugs that act at 5-HT2B or sigma receptors, but the clinical effect is uncertain.
What this interaction means in plain words
Fluoxetine and Gilteritinib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fluoxetine belongs to Central Nervous System; Gilteritinib belongs to Oncology.
Why it happens
Additive QT prolongation, with possible pharmacodynamic receptor-level interference.
What to do
Avoid if another antidepressant can be used. Otherwise monitor ECG and electrolytes and reduce or interrupt gilteritinib if QTc becomes markedly prolonged.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
