Serotonin syndrome has been reported when 5-HT3 antagonists are used with SSRIs such as fluoxetine. Fluoxetine also has a small QT effect that may add to that of granisetron. The overall absolute risk is low.
What this interaction means in plain words
Fluoxetine and Granisetron can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fluoxetine belongs to Central Nervous System; Granisetron belongs to Gastrointestinal Tract.
Why it happens
Possible additive serotonergic effect and modest additive QT prolongation.
What to do
Monitor for serotonin toxicity symptoms and consider an ECG in patients with other QT risk factors. Stop the serotonergic agents and give supportive care if syndrome features appear.
