Summary

Fluoxetine may increase voriconazole concentrations, and voriconazole may in turn raise fluoxetine levels. Both can contribute to QT prolongation, and higher exposure raises the risk of voriconazole toxicity such as visual disturbances, hepatotoxicity and arrhythmia.

What this interaction means in plain words

Fluoxetine and Voriconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fluoxetine belongs to Central Nervous System; Voriconazole belongs to Anti Fungals.

Evidence level: Low

Why it happens

Fluoxetine inhibits CYP2C19 and CYP3A4, and voriconazole inhibits CYP2C19, CYP2D6 and CYP3A4, so reciprocal metabolism is reduced.

What to do

Monitor ECG, liver function and voriconazole trough levels, and watch for fluoxetine adverse effects; consider an alternative antidepressant if QT risk is high.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
