Fluoxetine + Zolmitriptan

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Additive serotonergic effects may increase the risk of serotonin syndrome when an SSRI (fluoxetine) is used with a triptan (zolmitriptan); reported cases are rare but clinically important.

Mechanism of action

Pharmacodynamic: SSRIs increase synaptic serotonin; triptans are 5-HT1B/1D agonists. Combined serotonergic activity can precipitate serotonin toxicity. No meaningful pharmacokinetic interaction expected.

Clinical management

Do not routinely avoid; use the lowest effective triptan dose and counsel patients. Monitor closely at initiation and after dose changes for serotonin-toxicity symptoms (agitation, diaphoresis, tremor, hyperreflexia/clonus, fever). Avoid adding other serotonergic agents or MAOIs. If serotonin syndrome is suspected, stop serotonergic drugs and provide supportive care.
