Summary

Fluphenazine is a phenothiazine antipsychotic that can prolong the QT interval and has anticholinergic activity. Salbutamol may add to arrhythmia risk through tachycardia and hypokalemia, and ipratropium may add minor anticholinergic effects. The overall risk is mostly relevant in patients with other QT risk factors or when salbutamol is used at high doses.

What this interaction means in plain words

Fluphenazine and Ipratropium-salbutamol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fluphenazine belongs to Central Nervous System; Ipratropium-salbutamol belongs to Respiratory.

Evidence level: Low

Why it happens

Additive QT prolongation and tachycardia, plus additive antimuscarinic effects from ipratropium with limited systemic exposure.

What to do

Monitor ECG and potassium in patients with additional QT risk factors, and limit salbutamol to the lowest effective dose. Watch for anticholinergic effects such as constipation and urinary retention.
