Summary
Phenothiazines block alpha-adrenergic receptors and can cause vasodilation, so with a beta-agonist such as isoproterenol the hypotensive effect and reflex tachycardia may be exaggerated. Isoproterenol can also provoke arrhythmias, which may be more likely in patients with QT-prolonging drugs on board.
What this interaction means in plain words
Fluphenazine and Isoproterenol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fluphenazine belongs to Central Nervous System; Isoproterenol belongs to Respiratory.
Why it happens
Alpha-adrenergic blockade by fluphenazine combined with beta-adrenergic vasodilation, plus additive proarrhythmic potential.
What to do
Monitor blood pressure, heart rate and ECG during isoproterenol therapy, and correct electrolyte disturbances. Titrate isoproterenol carefully in patients on fluphenazine.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
