Summary
Idelalisib is a strong CYP3A inhibitor and can substantially raise salmeterol and fluticasone exposure. This increases the risk of tachycardia, QT prolongation and systemic corticosteroid effects.
What this interaction means in plain words
Fluticasone-salmeterol and Idelalisib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fluticasone-salmeterol belongs to Respiratory; Idelalisib belongs to Oncology.
Why it happens
Strong CYP3A4 inhibition decreasing clearance of salmeterol and fluticasone.
What to do
Avoid the combination and consider beclomethasone as the inhaled corticosteroid and a non-CYP3A4-dependent bronchodilator. If used together, monitor ECG and for Cushingoid effects.
What to ask your pharmacist
