Summary

Itraconazole is a strong CYP3A4 inhibitor and raises exposure to both fluticasone furoate and vilanterol. This may increase corticosteroid systemic effects including adrenal suppression, and beta-agonist cardiovascular effects. Itraconazole also prolongs the QT interval.

What this interaction means in plain words

Fluticasone-vilanterol and Itraconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Fluticasone-vilanterol belongs to Respiratory; Itraconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Strong CYP3A4 inhibition reduces clearance of both inhaled components, with additive QT effects.

What to do

Use with caution and monitor for cardiovascular effects, hypokalemia and QTc, and signs of systemic corticosteroid effects; consider an alternative antifungal or inhaler for prolonged use.
