Summary
Labetalol is a non-selective beta-blocker with alpha-blocking activity that can antagonize vilanterol's bronchodilation and may precipitate bronchospasm in patients with asthma or COPD. This can reduce control of airway disease and the effectiveness of rescue beta-agonists.
What this interaction means in plain words
Fluticasone-vilanterol and Labetalol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fluticasone-vilanterol belongs to Respiratory; Labetalol belongs to Cardiovascular.
Why it happens
Pharmacodynamic antagonism at beta2-adrenoceptors in the airways.
What to do
Prefer a cardioselective beta-1 blocker or a non-beta-blocker antihypertensive in patients with reactive airway disease. If labetalol is needed, monitor respiratory status closely.
