Overview
Additive bleeding risk due to SSRI-associated platelet dysfunction combined with valproate-related thrombocytopenia/platelet impairment.
Mechanism of action
Pharmacodynamic: SSRIs reduce intraplatelet serotonin and impair aggregation; valproate can cause thrombocytopenia and inhibit platelet function.
Clinical management
Assess bleeding risk; monitor for bruising/epistaxis/GI bleeding; consider baseline/periodic platelet counts if clinically indicated; minimize concurrent NSAIDs/anticoagulants; consider alternative antidepressant if high bleeding risk.
