Summary
Warfarin and fondaparinux both reduce clotting through different pathways, so combined use increases bleeding risk. Overlap is intentional when bridging, but fondaparinux does not meaningfully alter the INR, and continuing it after the INR is therapeutic exposes the patient to unnecessary bleeding.
What this interaction means in plain words
Fondaparinux and Warfarin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fondaparinux belongs to Cardiovascular; Warfarin belongs to Cardiovascular.
Why it happens
Additive anticoagulation from factor Xa inhibition and reduced vitamin K-dependent clotting factor synthesis.
What to do
Use overlap only for bridging and stop fondaparinux once the INR is therapeutic for the required period. Monitor INR and watch for bleeding.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
