Summary

Combining an ACE inhibitor with an mTOR inhibitor such as temsirolimus increases the risk of angioedema. Swelling of the face, lips, tongue or airway can be serious.

What this interaction means in plain words

Fosinopril and Temsirolimus can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Fosinopril belongs to Cardiovascular; Temsirolimus belongs to Oncology.

Evidence level: Moderate

Why it happens

Both drugs impair bradykinin breakdown: ACE inhibitors block kininase II and mTOR inhibitors may reduce dipeptidyl peptidase IV activity.

What to do

Counsel patients to seek urgent care for any swelling of the face, lips or tongue. Stop fosinopril and consider an alternative antihypertensive class if angioedema occurs.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
