Summary
Adding a loop diuretic to an ACE inhibitor and thiazide can cause marked volume depletion, with hypotension and acute kidney injury, especially at initiation or in volume-depleted patients. Electrolyte disturbances are also more likely.
What this interaction means in plain words
Fosinopril-hydrochlorothiazide and Furosemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fosinopril-hydrochlorothiazide belongs to Cardiovascular; Furosemide belongs to Cardiovascular.
Why it happens
Additive natriuresis and reduced glomerular filtration pressure from ACE inhibition in volume-depleted states.
What to do
Monitor blood pressure, renal function and electrolytes after starting or increasing furosemide. Consider lowering the diuretic dose if hypotension or rising creatinine occurs.
What to ask your pharmacist
