Summary
NSAIDs such as lornoxicam blunt the antihypertensive effect of ACE inhibitors and diuretics. Together with these drugs they raise the risk of acute kidney injury, especially in volume-depleted or elderly patients, and may cause hyperkalemia.
What this interaction means in plain words
Fosinopril-hydrochlorothiazide and Lornoxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fosinopril-hydrochlorothiazide belongs to Cardiovascular; Lornoxicam belongs to Musculoskeletal.
Why it happens
Inhibition of renal prostaglandin synthesis causes sodium retention and reduced renal perfusion, opposing diuretic and ACE inhibitor effects and impairing glomerular filtration.
What to do
Monitor blood pressure, serum creatinine and potassium, and ensure adequate hydration. Use the lowest effective NSAID dose for the shortest time or choose an alternative analgesic.
