Summary

Fostemsavir's active metabolite inhibits hepatic uptake transporters OATP1B1 and OATP1B3, which may raise systemic exposure to statins that depend on these transporters. Lovastatin acid is an OATP substrate, so higher levels could increase the risk of myopathy.

What this interaction means in plain words

Fostemsavir and Lovastatin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fostemsavir belongs to Anti Virals; Lovastatin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Inhibition of OATP1B1/1B3-mediated hepatic uptake of lovastatin acid reduces its hepatic clearance.

What to do

Use the lowest effective lovastatin dose and monitor for muscle pain, weakness or elevated CK. Consider a statin less dependent on OATP transport if symptoms develop.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
