Summary
Furosemide adds to the diuretic effect of hydrochlorothiazide, which can cause marked volume depletion, hypotension, renal impairment and electrolyte loss. Losartan may offset potassium loss but increases the risk of hypotension and kidney injury if the patient is volume depleted.
What this interaction means in plain words
Furosemide and Hydrochlorothiazide-losartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Furosemide belongs to Cardiovascular; Hydrochlorothiazide-losartan belongs to Cardiovascular.
Why it happens
Sequential nephron blockade producing additive natriuresis and potassium wasting, with ARB-reduced glomerular filtration pressure.
What to do
Monitor blood pressure, renal function, sodium and potassium after starting or changing doses. Reduce diuretic doses if volume depletion develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
