Summary

Galantamine enhances cholinergic activity while trimipramine blocks muscarinic receptors, so the two may oppose each other and reduce the benefit of galantamine in dementia. Both can also cause bradycardia or conduction effects, and galantamine is partly metabolized by CYP2D6, which tricyclics may inhibit.

What this interaction means in plain words

Galantamine and Trimipramine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Galantamine belongs to Central Nervous System; Trimipramine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism between a cholinesterase inhibitor and an anticholinergic tricyclic, with possible CYP2D6 competition.

What to do

Avoid anticholinergic tricyclics in patients on cholinesterase inhibitors where possible and choose a less anticholinergic antidepressant. If used together, monitor cognition and heart rate.

What to ask your pharmacist
