Summary
Mavacamten is cleared mainly by CYP2C19 with a contribution from CYP3A4. Gemfibrozil and its glucuronide inhibit several CYP enzymes, so mavacamten exposure may rise. Excess mavacamten can depress left ventricular ejection fraction and cause systolic heart failure.
What this interaction means in plain words
Gemfibrozil and Mavacamten are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Gemfibrozil belongs to Cardiovascular; Mavacamten belongs to Cardiovascular.
Why it happens
Inhibition of CYP2C19-mediated metabolism of mavacamten increases its systemic exposure.
What to do
Avoid the combination and use an alternative lipid-lowering agent. If treatment cannot be changed, monitor echocardiographic LVEF closely and adjust the mavacamten dose per labeling.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
