Summary
Gemfibrozil raises elagolix plasma concentrations substantially. Higher elagolix exposure increases the risk of dose-related adverse effects such as bone mineral density loss, hepatic transaminase elevation and mood changes. Labeling lists strong OATP1B1 inhibitors as contraindicated.
What this interaction means in plain words
Elagolix and Gemfibrozil are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Elagolix belongs to Musculoskeletal; Gemfibrozil belongs to Cardiovascular.
Why it happens
Gemfibrozil inhibits hepatic OATP1B1 uptake of elagolix, reducing its hepatic clearance and increasing systemic exposure.
What to do
Do not co-administer elagolix with gemfibrozil. Choose a different lipid-lowering agent such as a statin or fenofibrate, or an alternative therapy for endometriosis.
