Summary
Fluoroquinolones can cause hypoglycemia or hyperglycemia in patients on antidiabetic drugs. With repaglinide, an insulin secretagogue, hypoglycemia may be more likely.
What this interaction means in plain words
Gemifloxacin and Repaglinide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Gemifloxacin belongs to Anti Bacterial; Repaglinide belongs to Endocrine.
Why it happens
Fluoroquinolones alter pancreatic beta-cell insulin release, which adds to the insulin-secreting effect of repaglinide.
What to do
Monitor blood glucose closely during and after the antibiotic course and educate the patient on hypoglycemia symptoms. Reduce the repaglinide dose or change the antibiotic if glucose is unstable.
What to ask your pharmacist

- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
