Summary

Gentamicin can cause renal potassium and magnesium wasting, and salbutamol shifts potassium into cells. Together they may lead to hypokalemia that is more pronounced in patients receiving high-dose or nebulized salbutamol. Hypokalemia can promote arrhythmias and muscle weakness.

What this interaction means in plain words

Gentamicin and Ipratropium-salbutamol have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Gentamicin belongs to Anti Bacterial; Ipratropium-salbutamol belongs to Respiratory.

Evidence level: Low

Why it happens

Additive lowering of serum potassium through aminoglycoside renal tubular wasting and beta-2 mediated cellular uptake.

What to do

Monitor potassium and magnesium during gentamicin therapy, especially with frequent salbutamol use, and replace deficits as needed.
