Summary

Posaconazole is a strong CYP3A4 inhibitor, and azole antifungals can also raise sulfonylurea levels. Pioglitazone is partly metabolized by CYP3A4, and glimepiride by CYP2C9, so glucose-lowering effects and hypoglycemia risk may increase.

What this interaction means in plain words

Glimepiride-pioglitazone and Posaconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Glimepiride-pioglitazone belongs to Endocrine; Posaconazole belongs to Anti Fungals.

Evidence level: Low

Why it happens

Inhibition of CYP3A4 (and possibly CYP2C9) reduces metabolism of pioglitazone and glimepiride.

What to do

Monitor blood glucose during azole therapy and reduce the glimepiride dose if hypoglycemia develops.
