Summary
Zonisamide is a weak carbonic anhydrase inhibitor and can lower serum bicarbonate, causing a non-anion gap hyperchloremic metabolic acidosis. Metformin carries a risk of lactic acidosis, so a pre-existing acidotic tendency may increase the danger of that complication, particularly in patients with renal impairment or hypoxia.
What this interaction means in plain words
Glipizide-metformin and Zonisamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Glipizide-metformin belongs to Endocrine; Zonisamide belongs to Central Nervous System.
Why it happens
Carbonic anhydrase inhibition by zonisamide reduces bicarbonate and adds to the acid-base risk associated with metformin.
What to do
Check baseline and periodic serum bicarbonate and renal function during co-therapy. Reassess the need for metformin or zonisamide if persistent acidosis develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
