Summary
Lomitapide is a substrate of CYP3A4, and glyburide is a weak CYP3A4 inhibitor that may modestly raise lomitapide exposure. Higher lomitapide levels increase the risk of liver enzyme elevation, hepatic fat accumulation and gastrointestinal adverse effects.
What this interaction means in plain words
Glyburide and Lomitapide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Glyburide belongs to Endocrine; Lomitapide belongs to Cardiovascular.
Why it happens
Weak inhibition of CYP3A4-mediated lomitapide metabolism by glyburide.
What to do
Monitor liver enzymes and bilirubin per lomitapide labeling and keep the lomitapide dose within the labeled limit for patients on weak CYP3A4 inhibitors. Reduce the lomitapide dose if transaminases rise.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Can I take Glyburide with Lomitapide?
MediHelp rates the interaction as moderate. Glyburide and Lomitapide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
How serious is the interaction between Glyburide and Lomitapide?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor liver enzymes and bilirubin per lomitapide labeling and keep the lomitapide dose within the labeled limit for patients on weak CYP3A4 inhibitors. Reduce the lomitapide dose if transaminases rise.
