Summary

Hemin is used to treat acute porphyria attacks, and corticosteroids can worsen porphyria or blunt the benefit of hemin. Corticosteroids may induce hepatic delta-aminolevulinic acid synthase, increasing porphyrin precursor production.

What this interaction means in plain words

Hemin and Triamcinolone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Hemin belongs to Miscellaneous; Triamcinolone belongs to Endocrine.

Evidence level: Low

Why it happens

Corticosteroids may induce hepatic ALA synthase, opposing the suppression of heme synthesis produced by hemin.

What to do

Avoid corticosteroids in acute porphyria unless clearly needed. If used, monitor clinical response and urinary porphobilinogen/ALA.

What to ask your pharmacist
