Summary

Estrogens can precipitate attacks of acute porphyria by inducing hepatic heme synthesis. Estrogen therapy may therefore counteract the benefit of hemin in a patient with porphyria.

What this interaction means in plain words

Bazedoxifene-conjugated estrogens and Hemin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Bazedoxifene-conjugated estrogens belongs to Endocrine; Hemin belongs to Miscellaneous.

Evidence level: Low

Why it happens

Estrogens may induce delta-aminolevulinic acid synthase, increasing porphyrin precursor production that opposes hemin's repression of this enzyme.

What to do

Avoid estrogen-containing therapy in patients with acute porphyria where possible. If used, monitor for porphyria symptoms and porphyrin precursor levels.
