Summary
Lofexidine lowers blood pressure and heart rate, and hydrochlorothiazide adds to the blood pressure lowering. Thiazide-induced hypokalemia or hypomagnesemia may also add to lofexidine's QT-prolonging potential.
What this interaction means in plain words
Hydrochlorothiazide and Lofexidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide belongs to Cardiovascular; Lofexidine belongs to Central Nervous System.
Why it happens
Additive hypotensive effect (pharmacodynamic), plus electrolyte disturbance that may worsen QT risk.
What to do
Monitor blood pressure, heart rate and electrolytes, correcting potassium and magnesium. Reduce the dose or adjust the antihypertensive regimen if symptomatic hypotension or bradycardia occurs.
What to ask your pharmacist
