Summary
NSAIDs such as ibuprofen blunt the blood-pressure-lowering effect of thiazides and ARBs by promoting sodium and water retention. In volume-depleted or elderly patients, the triple effect on renal hemodynamics can cause acute kidney injury and hyperkalemia.
What this interaction means in plain words
Hydrochlorothiazide-irbesartan and Ibuprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-irbesartan belongs to Cardiovascular; Ibuprofen belongs to Musculoskeletal.
Why it happens
Inhibition of vasodilatory renal prostaglandins reduces glomerular perfusion and natriuresis while ARB and diuretic compromise compensatory renal autoregulation.
What to do
Avoid regular NSAID use where possible and prefer paracetamol. If needed, use the lowest dose for the shortest time and monitor blood pressure, creatinine and potassium.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
