Summary
Both salmeterol and hydrochlorothiazide can lower serum potassium, and the effects can be additive, particularly with high doses of the beta-agonist. Hypokalemia may provoke arrhythmias, especially in patients with heart disease or hypoxia.
What this interaction means in plain words
Hydrochlorothiazide-irbesartan and Salmeterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-irbesartan belongs to Cardiovascular; Salmeterol belongs to Respiratory.
Why it happens
Beta-2 stimulation shifts potassium into cells while the thiazide increases urinary potassium loss, producing additive hypokalemia.
What to do
Monitor serum potassium in patients with risk factors, such as heart disease or high-dose beta-agonist use. Replace potassium as needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
