Summary
Sulindac may blunt the antihypertensive effect of the ACE inhibitor and thiazide and increase the risk of acute kidney injury and hyperkalemia, although its renal effects may be somewhat less than other NSAIDs.
What this interaction means in plain words
Hydrochlorothiazide-lisinopril and Sulindac can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-lisinopril belongs to Cardiovascular; Sulindac belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis causes sodium and water retention and reduced glomerular perfusion, which opposes the antihypertensive and diuretic effects.
What to do
Monitor blood pressure, serum creatinine, and potassium, and keep NSAID therapy brief. Ensure adequate hydration.
