Summary
Lornoxicam, like other NSAIDs, can weaken the antihypertensive effect of losartan and hydrochlorothiazide. It also increases the risk of renal impairment and hyperkalemia, particularly in older or volume-depleted patients.
What this interaction means in plain words
Hydrochlorothiazide-losartan and Lornoxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-losartan belongs to Cardiovascular; Lornoxicam belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion, opposing the ARB and diuretic and impairing renal function.
What to do
Use the lowest effective NSAID dose for the shortest duration and ensure hydration. Monitor blood pressure, creatinine and potassium.
