Summary
NSAIDs such as nabumetone can blunt the blood pressure lowering effect of losartan and hydrochlorothiazide. In patients who are older, volume depleted or have reduced kidney function, the triple effect of an ARB, a diuretic and an NSAID can cause acute kidney injury and hyperkalemia.
What this interaction means in plain words
Hydrochlorothiazide-losartan and Nabumetone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-losartan belongs to Cardiovascular; Nabumetone belongs to Musculoskeletal.
Why it happens
NSAIDs inhibit synthesis of vasodilatory renal prostaglandins, reducing renal perfusion and sodium excretion and opposing ARB and diuretic effects.
What to do
Monitor blood pressure, serum creatinine and potassium after starting or changing the NSAID, and ensure adequate hydration. Use the lowest effective NSAID dose for the shortest time or choose an alternative analgesic such as paracetamol.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
