Summary
Adding torsemide to a thiazide-containing regimen produces additive diuresis, which can lead to volume depletion, hypotension, hypokalemia, hyponatremia and renal impairment. Losartan may partly offset potassium loss but also raises the risk of renal dysfunction when volume depleted.
What this interaction means in plain words
Hydrochlorothiazide-losartan and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-losartan belongs to Cardiovascular; Torsemide belongs to Cardiovascular.
Why it happens
Sequential nephron blockade by loop and thiazide diuretics gives additive natriuresis and potassium loss, with angiotensin receptor blockade adding hemodynamic effects.
What to do
Monitor electrolytes, creatinine, weight and blood pressure after starting or changing doses. Reduce the diuretic dose if volume depletion or electrolyte disturbance occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
