Summary
Both drugs can prolong the QT interval, and ibutilide carries a substantial risk of torsades de pointes on its own. Adding oxaliplatin, which has been associated with QT prolongation and can cause electrolyte disturbances, further increases arrhythmia risk.
What this interaction means in plain words
Ibutilide and Oxaliplatin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Ibutilide belongs to Cardiovascular; Oxaliplatin belongs to Oncology.
Why it happens
Additive prolongation of cardiac repolarization (QT interval) through pharmacodynamic effects.
What to do
Avoid the combination where possible; if ibutilide is essential, monitor ECG continuously and correct potassium and magnesium beforehand. Consider an alternative antiarrhythmic strategy.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
