Summary
Ifosfamide is a prodrug activated by CYP3A4 and CYP2B6, and nevirapine induces both enzymes. This may increase formation of active alkylating metabolites and also of neurotoxic and nephrotoxic metabolites.
What this interaction means in plain words
Ifosfamide and Nevirapine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ifosfamide belongs to Oncology; Nevirapine belongs to Anti Virals.
Why it happens
Nevirapine induces CYP3A4 and CYP2B6, increasing ifosfamide bioactivation and chloroacetaldehyde formation.
What to do
Monitor for neurotoxicity, renal function and myelosuppression during ifosfamide cycles, and reduce the dose or stop if toxicity develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
