Summary

Stiripentol inhibits and induces several CYP enzymes, including CYP3A4 and CYP2C19, which take part in ifosfamide activation and in forming its neurotoxic and nephrotoxic metabolite. Exposure to active and toxic metabolites may therefore change unpredictably, altering efficacy or toxicity.

What this interaction means in plain words

Ifosfamide and Stiripentol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Ifosfamide belongs to Oncology; Stiripentol belongs to Central Nervous System.

Evidence level: Low

Why it happens

Stiripentol modulates CYP3A4 and CYP2C19, altering ifosfamide metabolic activation and toxic metabolite formation.

What to do

Monitor for ifosfamide neurotoxicity, urotoxicity and myelosuppression, and for loss of efficacy. Adjust the ifosfamide dose according to tolerance.

What to ask your pharmacist
