Summary
Linezolid is a weak reversible MAO inhibitor and iloperidone has serotonergic receptor activity, so combined use could rarely contribute to serotonin toxicity. Dopamine blockade may also add to neuroleptic malignant syndrome risk. The overall likelihood is low but the reactions are serious.
What this interaction means in plain words
Iloperidone and Linezolid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Iloperidone belongs to Central Nervous System; Linezolid belongs to Anti Bacterial.
Why it happens
Additive serotonergic effects from MAO inhibition by linezolid with antipsychotic 5-HT receptor activity and dopamine antagonism.
What to do
Monitor for agitation, tremor, clonus, hyperthermia, rigidity and autonomic instability during the linezolid course. Stop one or both agents if features of serotonin syndrome or NMS appear.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
