Summary

Iloperidone has been described as a weak time-dependent CYP3A inhibitor, so it may modestly raise nicardipine concentrations. This could increase hypotension, dizziness and reflex tachycardia. Both drugs can also lower blood pressure through additive effects, since iloperidone causes orthostatic hypotension via alpha-1 blockade.

What this interaction means in plain words

Iloperidone and Nicardipine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Iloperidone belongs to Central Nervous System; Nicardipine belongs to Cardiovascular.

Evidence level: Low

Why it happens

Iloperidone may inhibit CYP3A4 and adds alpha-1 adrenergic blockade to the vasodilation of nicardipine.

What to do

Monitor blood pressure, including orthostatic readings, when starting or adjusting either drug. Reduce the nicardipine dose if symptomatic hypotension develops.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
