Summary

Procarbazine has monoamine oxidase inhibiting activity and may add to the serotonergic and dopamine-modulating effects of antipsychotics. This could raise the risk of serotonin toxicity or neuroleptic malignant syndrome, and additive sedation and hypotension may also occur. The clinical evidence is limited and mostly theoretical.

What this interaction means in plain words

Iloperidone and Procarbazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Iloperidone belongs to Central Nervous System; Procarbazine belongs to Oncology.

Evidence level: Low

Why it happens

Weak MAO inhibition by procarbazine combined with antipsychotic receptor antagonism may produce additive serotonergic and dopaminergic effects.

What to do

Monitor for mental status changes, hyperthermia, rigidity, clonus and autonomic instability, particularly after starting or increasing doses. Stop both agents and give supportive care if serotonin syndrome or neuroleptic malignant syndrome is suspected.

What to ask your pharmacist
