Summary

Tipranavir given with ritonavir strongly inhibits CYP3A4 and may substantially increase imatinib levels. This increases the risk of imatinib toxicity such as edema, myelosuppression and hepatotoxicity. Tipranavir also has a mixed inducing effect, making the net result less predictable.

What this interaction means in plain words

Imatinib and Tipranavir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Imatinib belongs to Oncology; Tipranavir belongs to Anti Virals.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4 metabolism of imatinib by ritonavir-boosted tipranavir.

What to do

Prefer an alternative antiretroviral. If coadministered, monitor blood counts, liver function and fluid status and consider reducing the imatinib dose.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
