Summary
Modafinil may inhibit CYP2C19 and raise imipramine and desipramine exposure in some patients, particularly CYP2D6 poor metabolizers. This can increase anticholinergic and cardiac effects, while the two drugs have opposing effects on alertness.
What this interaction means in plain words
Imipramine and Modafinil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Imipramine belongs to Central Nervous System; Modafinil belongs to Central Nervous System.
Why it happens
Modafinil inhibition of CYP2C19 reduces imipramine demethylation.
What to do
Monitor for tricyclic adverse effects such as tachycardia, dry mouth and sedation, and measure tricyclic levels if toxicity is suspected. Reduce imipramine dose as needed.
What to ask your pharmacist
