Summary

NSAIDs such as lornoxicam can blunt the antihypertensive effect of irbesartan and, by reducing renal prostaglandins, impair kidney function and raise potassium. The risk of acute kidney injury is higher in older, volume-depleted or renally impaired patients.

What this interaction means in plain words

Irbesartan and Lornoxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Irbesartan belongs to Cardiovascular; Lornoxicam belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and potassium excretion, compounding angiotensin receptor blockade.

What to do

Ensure adequate hydration and monitor blood pressure, serum creatinine and potassium. Use the lowest effective NSAID dose for the shortest time, or choose an alternative analgesic.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
