Summary
Itraconazole is a strong CYP3A4 inhibitor and markedly increases vardenafil exposure, raising the risk of hypotension, priapism and QT prolongation. Itraconazole itself can affect cardiac repolarization. Labeling restricts the vardenafil dose with strong CYP3A4 inhibitors.
What this interaction means in plain words
Itraconazole and Vardenafil can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Itraconazole belongs to Anti Fungals; Vardenafil belongs to Genitourinary.
Why it happens
Inhibition of CYP3A4-mediated vardenafil metabolism, plus additive QT effects.
What to do
Avoid the combination where possible. If used, follow labeling with a markedly reduced vardenafil dose and longer dosing interval, and consider ECG monitoring.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
