Summary

Higher gastric pH can reduce absorption of posaconazole oral suspension, lowering antifungal exposure and risking treatment failure. Delayed-release tablets are much less affected. Posaconazole may also slightly raise lansoprazole levels, which is rarely important.

What this interaction means in plain words

Lansoprazole and Posaconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Lansoprazole belongs to Gastrointestinal Tract; Posaconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Reduced gastric acidity impairs dissolution of posaconazole suspension, while posaconazole weakly inhibits CYP3A4 metabolism of lansoprazole.

What to do

Prefer the delayed-release tablet or intravenous posaconazole with a PPI, or avoid the suspension; monitor posaconazole trough levels in prophylaxis or treatment of invasive fungal infection.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
