Summary

Larotrectinib is a CYP3A4 substrate and a weak inhibitor of CYP3A4 in vitro, so it could modestly raise nevirapine exposure. Any effect is expected to be small, but nevirapine toxicity such as rash and hepatotoxicity is exposure related. Nevirapine could in turn lower larotrectinib levels through induction.

What this interaction means in plain words

Larotrectinib and Nevirapine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Larotrectinib belongs to Oncology; Nevirapine belongs to Anti Virals.

Evidence level: Low

Why it happens

Possible weak CYP3A4 inhibition by larotrectinib reducing nevirapine metabolism; the effect is predicted and not well characterized.

What to do

Monitor liver function and skin reactions, and monitor larotrectinib efficacy; no routine dose change is needed.
