Summary
Lurasidone blocks dopamine D2 receptors and can counteract the antiparkinsonian effect of levodopa, worsening motor symptoms. Additive orthostatic hypotension may also occur.
What this interaction means in plain words
Levodopa and Lurasidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Levodopa belongs to Central Nervous System; Lurasidone belongs to Central Nervous System.
Why it happens
Pharmacodynamic antagonism at dopamine D2 receptors, with additive hypotensive effects.
What to do
Monitor motor function and blood pressure, and use the lowest effective lurasidone dose. In Parkinson's disease, prefer an antipsychotic with less D2 blockade such as quetiapine or clozapine.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
