Summary
Paliperidone blocks dopamine D2 receptors and can counteract the antiparkinsonian benefit of levodopa. This may worsen motor symptoms in Parkinson disease, while levodopa may in turn aggravate psychosis.
What this interaction means in plain words
Levodopa and Paliperidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Levodopa belongs to Central Nervous System; Paliperidone belongs to Central Nervous System.
Why it happens
Pharmacodynamic antagonism between D2 receptor blockade and dopamine replacement.
What to do
Avoid paliperidone in Parkinson disease; if an antipsychotic is needed, use quetiapine, clozapine or pimavanserin. Monitor motor function closely if the combination cannot be avoided.
