Summary

Levoketoconazole is a strong CYP3A4 inhibitor and raises nilotinib exposure. Both drugs also prolong the QT interval, so the risk of torsades de pointes is substantially increased.

What this interaction means in plain words

Levoketoconazole and Nilotinib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Levoketoconazole belongs to Endocrine; Nilotinib belongs to Oncology.

Evidence level: High

Why it happens

Strong CYP3A4 inhibition raises nilotinib concentrations, with additive QT prolongation.

What to do

Avoid the combination. If an alternative to either drug is impossible, reduce the nilotinib dose per labeling and monitor ECG closely.

What to ask your pharmacist
