Summary

Both drugs prolong the QT interval, and levoketoconazole also inhibits CYP3A4 and may raise amiodarone concentrations. The combined effect increases the risk of torsades de pointes and other serious ventricular arrhythmias.

What this interaction means in plain words

Amiodarone and Levoketoconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Amiodarone belongs to Cardiovascular; Levoketoconazole belongs to Endocrine.

Evidence level: Moderate

Why it happens

Additive QT prolongation plus CYP3A4 inhibition by levoketoconazole reducing amiodarone metabolism.

What to do

Avoid concurrent use; choose an alternative antiarrhythmic or cortisol-lowering therapy. If unavoidable, obtain ECG monitoring and correct potassium and magnesium.
