Summary
Levoketoconazole is a strong CYP3A4 inhibitor that can raise clarithromycin levels, and both drugs prolong the QT interval. Levoketoconazole is contraindicated with other QT-prolonging CYP3A4 substrates, so the combination carries a serious arrhythmia risk.
What this interaction means in plain words
Clarithromycin and Levoketoconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Clarithromycin belongs to Anti Bacterial; Levoketoconazole belongs to Endocrine.
Why it happens
CYP3A4 inhibition increases clarithromycin exposure, with additive QT prolongation.
What to do
Do not co-administer; use a non-QT-prolonging antibiotic that is not a CYP3A4 substrate or inhibitor.
